Underworld Grill and Bar operated in Las Vegas under owners and musicians Jazz Bender and Kris Kaos, who bought the horror-themed bar in 2011 and built its concept around a pizza menu, according to Reality TV Updates. By the time of the Bar Rescue visit, the bar had only two to three months of runway left before an expected closure. Jon Taffer's team found no draft beer service despite an existing tap system, a soggy and poorly made pizza served to undercover patrons, and a bartender whose approach to attracting customers repelled them instead; the owners also distrusted their own staff going into the visit.

As part of the rescue, mixologist Russell Davis retrained the bar program while pizza expert Tony Gemignani worked with the kitchen on proper technique and higher-quality ingredients. The owners initially resisted a name change but agreed after hearing Taffer's concept. The decor was rebuilt around a zombie and post-apocalypse theme, and the bar was renamed from Underworld Grill and Bar to The End.

Sales were reported up roughly 40% eight weeks after the visit, and the working relationship between staff and ownership also improved, according to Reality TV Updates. However, the bar later had to relocate a short distance, moving from its original Decatur Boulevard address to a nearby Spring Mountain Road strip-mall space in March 2014; a contemporary Yelp review cited by Reality TV Updates attributed the move to a roof collapse at the original building. The relocation meant the business could not carry over the structural elements of Taffer's renovation, though it kept much of its equipment and zombie-themed branding.
