PJ's Steakhouse occupied a corner spot on Yellowstone Boulevard in the Forest Hills section of Queens when it appeared in Kitchen Nightmares' third episode of Season 4, filmed in spring 2010 and aired February 4, 2011. Husband-and-wife owners Joe and Madalyn had sunk close to two million dollars into the restaurant, which Joe opened partly as a tribute to his late brother PJ, a former bartender at the same address. Neither owner had run a restaurant before, and the strain showed: Ramsay found the dining room empty, Joe drinking at the bar during service rather than managing the floor, and head chef Eric turning out inconsistent food, including a crab cake with a piece of plastic in it and a filet Ramsay judged tough. For a restaurant built around steak, the menu offered only a couple of cuts and no strip steak, a gap Ramsay called out directly. Madalyn told Ramsay the restaurant brought in about $4,000 a week against roughly $17,000 in costs to stay open, and that the couple had sold their custom-built house to keep the business afloat. During a test dinner service, wait times stretched toward two hours and dishes came back cold; when Ramsay introduced a mixed grill special, Chef Eric walked off the line rather than fix the pacing, forcing the kitchen to close for the night.

Ramsay's overhaul went beyond the menu: he redesigned the space around a tribute to Joe's brother, brought in a new chef and a new general manager, and renamed the restaurant PJ's Grill to better match a simpler, focused steak-and-comfort-food concept. The relaunch drew a fuller dining room and calmer service than the restaurant had seen in months.

The turnaround did not hold. According to the episode's own closing update, PJ's Grill closed within weeks of Ramsay's visit and did not reopen before the episode aired on Fox nine months later. Joe returned to construction work, the industry he had worked in before opening the restaurant.
