The restaurant began as Rex 1516 on South Street in Philadelphia. The Philadelphia Inquirer reported on 2021-09-30 that the restaurant was relocating next door into the newly renovated Royal Theater building, a 1920s venue once known as a historic Black vaudeville and film house, and would reopen under the new name Rex at the Royal.
